As catalogued
Provider-led Bay Area health network, founded in 2015 by UCSF Health and John Muir Health, that co-designed simpler and more affordable health plans with insurers around a curated network of leading local providers.
2026 reality
Canopy Health (legal name Bay Area Accountable Care Network, Inc.) operates as a provider-owned accountable care network holding a restricted Knox-Keene licence from California's Department of Managed Health Care; it is the featured network in Health Net (CanopyCare HMO, offered e.g. to San Francisco city employees for 2026) and UnitedHealthcare plans. For 2026 it moved its management services organisation from Conifer to MedPoint Management and changed prior-authorisation requirements.
The original model remains substantially intact: Canopy is still a provider network sold through insurers' plans, not a risk carrier selling under its own brand. Its network and insurer partnerships grew (UnitedHealthcare added; Health Net CanopyCare launched in 2020).
a regional, provider-owned network model in which health systems co-design insurer plans; relevant as an example of provider-payer integration rather than as a digital insurtech.
Third taxonomy
REVIEW · Confirmed the January 2026 MSO switch to MedPoint on Canopy's providers page and the ownership via search (consistent with the 2020 Health Net release). Added UCSF sources for the March 2015 formation, the July 2016 rename and the 2016 Knox-Keene licence; the HCAI filing PDF could not be text-extracted here.
